Unlimited Resilience, LLC is an “out of network” private practice that does not accept insurance. However, we can provide “superbills” which can be submitted to your insurance for potential reimbursement, depending on your insurance plan policy.
Here’s some information to have ready and questions that can help when talking with your insurance company about your out-of-network benefits:
- Name and date of birth of client
- Name and date of birth of policyholder if you are not the policyholder (i.e. if you are on your parent or spouses’ insurance plan)
- Your insurance member ID number
- Your insurance group number
We recommend you ask for a call reference number for tracking purposes. You will also need the practice details listed for accurate information.
Provider Name/NPI: This will depend on the team member you are working with. Before you call your insurance, please reach out to the therapist to get their full name, credentials, and NPI #, and if applicable, the full name, credentials, NPI # of the supervising clinician.
